STUDIES

This trial will collect data on patients with Aorto-iliac occlusive disease, who underwent treatment with the BeGraft Aortic and Peripheral covered stent Graft in CERAB configuration.

• The devices under investigation already bare the CE mark for another indication and are being used outside of their intended use in this study.
• The current SOC is open surgery and CERAB procedures are performed with devices outside of their intended use.
• The study is constructed to evaluate the clinical safety and performance à Conformity assessment for CE-Certification

• Patient has been identified with an aorto-iliac occlusive lesion, with clinical necessity for treatment.
• The patient has been diagnosed with symptomatic peripheral artery disease, defined by Rutherford Becker
• Classification score 2 to 5.
• Patient has a projected life-expectancy of at least 24 months.
• Patient is ≥18 years old.
• Patient is willing/capable and provides written consent to participate to the trial and confirmed to attend the expected follow-up visits.

• Patient is currently participating in another investigational drug or device trial that has not completed the entire
• follow up period.
• Patient has planned any surgical intervention/procedure within 30 days after the study procedure.
• Patient had a Myocardial infarction or stroke within a period of 3 months prior to the study procedure.
• Patient had surgery (e.g. bypass surgery or stenting) in target vessels previously.
• Patient with acute aortic or iliac occlusive disease.
• Patient has an acute systemic infection at time of screening or in period of 30 days prior to screening.
• Patient has fresh thrombus at time of screening or in period of 14 days prior to screening.
• Patient has a CERAB procedure that is staged.
• Female patient with childbearing potential not taking adequate contraceptives or currently breastfeeding.
• Patients for whom antiplatelet therapy, anticoagulants or thrombolytic drugs are contraindicated.
• Patients with known hypersensitivity to the stent material (L605) and/or PTFE.
• Patients who are placed in an institution due to an institutional or court order.

The aim is to evaluate safety, tolerability and pharmacokinetic profile (PK: both systemic and local vascular injury site-specific PK) and to determine the biodistribution and internal radiation dosimetry of the tracer dose of [89 Zr]ZrDFO-APAC. In addition, the binding and retention time of [89 Zr]Zr-DFO-APAC to arteries and atherosclerotic or microvascular lesions will be investigated.

• Males and females aged 40-85 years
• PAOD/CLI patients Rutherford categories 1-4 and category 5 Wlfl wound grade of 0 or 1
• Estimated glomerular filtration rate (eGFR) > 46 ml/min/1.73 m2 as per calculation of Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI)
• CT angiography with contrast agent performed within 3 months of the patients’ first PEY-scan as part of diagnostics of PAOD, with results available in the subject’s medical records
• No surgical or endovascular interventions for PAOD within 1 year of the first PET/CT-scan or planned between inclusion and the patients’ last PET-scan
• Provision of valid informed consent and capability to communicate well with the investigator
• Pre-menopausal women must be willing to practice highly effective contraception for 195 days after IMP administration
• Men must be willing to practice highly effective contraception for 105 days after IMP administration, including condom use during the first 15 days after the IMP administration to prevent transmission of 89-Zr to a partner of childbearing potential
• Patients should be able to understand all study-related information in Dutch

• An existing aneurysm that requires surgical intervention.
• Medical history of, or condition known to be associated with impaired hemostasis, such as an increased intracranial bleeding risk e.g., previous history of intracranial hemorrhage, subarachnoidal bleeding, hemorrhagic stroke, or gastrointestinal or retroperitoneal bleeding, or any inherited or acquired bleeding disorder, such as von Willebrand disease or hemophilia.
• Any cerebrovascular event (including transient ischemic attack, thrombotic or embolic stroke) within the past year.
• Diagnosis of autoimmune (Type 1, or latent autoimmune diabetes in adults (LADA)) diabetes mellitus.
• HbAlc > 10% at screening.
• Current use of anticoagulant therapy (warfarin, apixaban, rivaroxaban, dabigatran, edoxaban, fondaparinux, or any heparin derivative) for any medical reason.
• Patients treated with combined antiplatelet agents, excluding a single agent, such as acetylsalicylic acid (up to 100 mg QD) or clopidogrel (up to 75 mg QD).
• Use of non-steroidal anti-inflammatory medications within 2 weeks prior to dosing with [89Zr]Zr-DFO-APAC. If pain relief is required, paracetamol may be used.
• Use of selective serotonin reuptake inhibitor (SSRI) medications within 2 weeks prior to dosing with [89Zr]Zr-DFO-APAC.
• Major surgery, major trauma or any endovascular intervention within the past 90 days or organ biopsy or diagnostic angiography within the past 30 days prior to the screening visit.
• Uncontrolled arterial hypertension (persistent systolic blood pressure > 160 mmHg or diastolic blood pressure >100 mmHg).
• Hemoglobin < 8.0 mmol/L (130 g/I) (men) or <7.5 mmol/L (120 g/l) (women) at screening, or platelet count < 150 x 109/L and leukocyte count >12 x 109/L.
• Clinically significantly prolonged plasma prothrombin time (PT) (> 1.2-fold) or an
• International normalixed ratio (INR) of > 1.2
• Activated plasma partial thromboplastin time (APT!) above the upper limit of the reference range.
• Patients with a medical history of heparin-induced thrombocytopenia.
• Patients with known significant liver disease, incl. an alanine aminotransferase (ALT) and/or aspartate aminotransferase (AST) level > 2.5 x ULN at screening.
• A diagnosis of severe chronic kidney disease, defined as having a glomerular filtration rate (GFR) category 4 or 5 (GFR 45mUmin/1.73 m2) and albuminuria stage A3
• albumin:creatinine ratio (ACR) of > 300 mg/g).
• Patients with an active malignancy or who have received treatment for any malignancy within 1 year before the screening visit, except for localized basal cell or squamous cell skin cancer that has been cured at least 90 days before screening.
• Patients with a history of metastatic malignancy.
• Women who are pregnant or lactating.
• Patients with a known allergy or hypersensitivity to heparin or heparin products, and/or antiplatelet agents (e.g., aspirin or clopidogrel).
• Participation in an investigational drug or device study within 30 days prior to screening.
• Patients who have ever received treatment with an antibody or gene therapy product or participated in a clinical trial with such agents (unless there is evidence that they received placebo only).
• Patients with known antiphospholipid antibody syndrome or other thrombophilia
• Patients with a medical history of severe infection or vasculitis due to an autoimmune disease, inflammatory bowel disease (IBD) or Crohn's disease.
• Any concomitant disease or condition that could interfere with, or the treatment of which might interfere with, the conduct of the study, or that would, in the opinion of the investigator, pose an unacceptable risk to the subject.
• Patients unable or unwilling to comply with the protocol or to cooperate fully with the investigator or site personnel.
• Patients with anamnestic history of drug abuse (defined as illicit drug use) or anamnestic history of alcohol abuse (defined as regular or daily consumption of more than 4 alcoholic drinks per day) within the past 12 months.

• Male or female volunteers 40-80 years old, inclusive.
• Healthy status defined by the absence of evidence of any active or chronic disease following a detailed medical and surgical history, a complete physical examination including vital signs, 12-lead ECG, hematology, blood chemistry, serology and urinalysis.
• Normal fibrinogen (2.0 - 4.0 g/L) and D-dimer 0.5 mg/L FEU) levels.
• No diagnosis of diabetes, pre-diabetes or insulin resistance.
• Normal eGFR as per calculation of Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI).
• Premenopausal woman must be willing to practise highly effective contraception for 195 days after IMP administration.
• Men must be willing to practise highly effective contraception for 105 days after IMP administration, including condom use during the first 15 days after the IMP administration

In deze studie wordt monotherapie clopidogrel vergeleken met duale therapie bestaande uit clopidogrel/acetylsalicylzuur. Deze plaatjesremmers worden al decennia lang voorgeschreven na een dotterbehandeling. Dit onderzoek test daarom geen nieuwe medicijnen maar heeft als doel de effectiviteit van al bestaande plaatjesremmers of een combinatie daarvan te toetsen.

• Laesie van de a. iliaca/femoralis/poplitea en/of arteriën onder de knie (BTK)
• Alle TASC-laesie
• Rutherford 1-6 categorieën met indicatie voor interventie
• Patiënt moet de informatie goed begrijpen
• Getekend informed consent door de patiënt
• Leeftijd >/= 45 jaar
• Laesie is geschikt voor PTA/rekanalisatie met of zonder stentplaatsing, of;
• Hybride procedure met endarterectomie van de arterie femoralis met PTA, of;
• Patiënten met een reïnterventie binnen 2 maanden door een gefaseerde behandeling

• Acute ischemie van de onderste extremiteiten
• Gerapporteerde intolerantie/hypersensitiviteit voor de studie medicatie
• Gebruik van anticoagulantia (DOAC/vitamine K antagonist)
• Momenteel > 2 weken gebruik van een NSAID welke niet gestopt kan worden
• Patiënt kan de informatie niet goed begrijpen
• Patiënt met een reïnterventie door een restenose/reocclusie binnen 2 maanden
• Patiënt met een hybride procedure anders dan endarterectomie van de a. femoralis zoals a. femoralis bypass met PTA
• Patiënten met een stollingsstoornis
• Patiënten met een ulcus pepticum in de voorgeschiedenis welke bevestigd is middels een gastroscopie
• Zwangere, patiënten met een zwangerschapswens of die borstvoeding geven
• Dialyse behoeftige patiënten
• Patiënten met leverfunctiestoornis met minimaal één van de volgende criteria; verhoogd INR, portale hypertensie, trombocytopenie (<50x109/L), de bovenstaande gegevens onbekend zijn.

The PREDICT project aims to increase knowledge and competencies on percutaneous PD catheter insertions by training centers for this technique. By establisching a prospective registry, the outcomes of percutaneously inserted PD catheters, both in experienced and newly trained centers, will be assessed.

• The inclusion criteria for the prospective PREDICT registry are age > 18 years and percutaneous PD catheter insertion between 1 March 2023 and 1 March 1 2026 in participating centers.
• Patients can be included 4 weeks before until 12 weeks after PD catheter insertion. The exclusion criteria are the inability to give informed consent and a life expectancy of less than 3 months.

Het doel van deze studie is het direct vergelijken van de standaard behandeling, I-131, met de nieuwe behandeling, RFA. Primaire uitkomstmaat is hypothyreoïdie. Daarnaast zal gekeken worden naar herstel van schildklierfunctie, volume reductie van de behandelde nodus, (schildklier gerelateerde) kwaliteit van leven, patiëntvoorkeuren en kosten-effectiviteit.

• Leeftijd >18 jaar
• Hyperthyreoïdie veroorzaakt door enkele autonoom functionerende schildkliernodus, aangetoond door:
• Onderdrukt TSH met of zonder verhoogd FT4/FT3
• Anti-TSH negatief
• Echografisch enkele nodus of multinodulair struma
• Scintigrafisch bewezen één autonome nodus incl uptake bepaling
• Behandeling I-131 geïndiceerd
• Behandeling RFA geschikt
• Informed consent

• Multifocale autonomie (toxisch multinodulair struma)
• Nodus > 5cm
• Nodus >75% cysteus gedegenereerd
• Medisch device gevoelig voor RFA currents (zoals pacemaker)
• Dysfagie of andere reden waarom I-131 niet ingenomen zou kunnen worden
• I-131 of RFA van zelfde nodus in voorgeschiedenis
• Oncorrigeerbare hemorragische diathese
• Zwangerschap of borstvoeding

Het doel van het onderzoek is om te bepalen of katheter-geassisteerde
trombolyse (CDT) bij hoog-risico longembolie (PE) ten opzichte van systemische
trombolyse:

• effectiever en veiliger is in termen van een vermindering van het
• samengestelde eindpunt van algehele sterfte en complicaties, gedefinieerd als
• behandelfalen, majeure bloeding en CVA, ongeacht de oorzaak op dag 30 (primaire
• uitkomstmaat)
• leidt tot een betere Desirability of Outcome Ranking (DOOR) op dag 7
• geassocieerd is met een lager niveau van zuurstoftoediening na 48 uur
• geassocieerd is met een kortere verblijfsduur (LOS) op de intensive care (IC)
• en in het ziekenhuis in het algemeen
• geassocieerd is met beter functioneel herstel evenals betere door de patiënt
• gerapporteerde uitkomsten zoals kwaliteit van leven (QoL) na één jaar
• kosteneffectief is na een tijdshorizon van één jaar

• Volwassen patiënten met een bevestigde acute longembolie (LE), d.w.z. een contrastvullingsdefect in een lobaire of meer proximale longslagader op computed tomography longangiografie (CTPA), en/of obstructieve shock met echocardiografisch bevestigde dilatatie van de rechterventrikel en een verstopte vena cava inferior, zowel met als zonder echocardiografische tekenen van een transporterend stolsel of diepe veneuze trombose van het been.

• Hoog mortaliteitsrisico, d.w.z.:

• Post cardiaal arrest (na tijdelijke noodzaak voor cardiopulmonaire reanimatie)

• OF Obstructieve shock (systolische bloeddruk <90 mmHg en tekenen van eind-orgaan hypoperfusie bijvoorbeeld verhoogde lactaatniveaus >2 mmol/l) of de noodzaak voor vasopressoren (adrenaline of noradrenaline) om een adequate bloeddruk te handhaven

• OF Persisterende hypotensie (systolische bloeddruk <90 mmHg of een daling van de systolische bloeddruk >=40 mmHg gedurende ten minste 15 minuten) niet veroorzaakt door een nieuwe hartritmestoornis, hypovolemie of sepsis

• OF Abnormale rechterventrikelfunctie op transthoracale echocardiografie of CTPA
• EN Verhoogde cardiale troponineniveaus
• EN Ademhalingsfalen gedefinieerd als hypoxemie (SaO* <90%) refractair voor zuurstof suppletie via neuskanaal of Venturi-masker, wat vereist dat zuurstofsuppletie via een volledig gezichtsmasker (100% FiO2), hoogdoorstromend neussysteem zuurstof, of (niet-)invasieve mechanische ventilatie noodzakelijk is.

• CDT beschikbaar en technisch haalbaar zodat een randomisatie-naar-naald tijd van 60 minuten of minder mogelijk is.

• *Catastrofale LE*, d.w.z. aanhoudende hartstilstand en/of noodzaak voor extracorporele cardiopulmonale reanimatie (ECPR) en/of onmiddellijke indicatie voor venoarteriële extracorporele membraanoxygenatie (VA-ECMO) zoals beoordeeld door de verantwoordelijke arts(en).
• Glasgow Coma Scale <8 na reanimeren
• Alternatieve diagnose dan acute LE die aanzienlijk bijdraagt aan acuut hemodynamisch en/of respiratoir falen, bijv. sepsis, COPD GOLD 3 of 4, of bekend hartfalen met NYHA klasse 4, zoals beoordeeld door de behandelend arts.
• Bekende *geen opname op de IC* of *niet reanimeren* verklaring.
• Absolute contra-indicatie voor systemische thrombolyse, d.w.z.:
• - Hemorragisch CVA in de anamnese (i.a.),
• - Ischemisch CVA in de afgelopen 6 maanden,
• - Neoplasma van het centrale zenuwstelsel,
• - Hoog-Energetisch Trauma (HET), grote operatie of ernstig hoofdletsel in de afgelopen 3 weken (opmerking: milde externe snijwonden van het hoofd na bijv. syncope tellen niet als ernstig hoofdletsel, vooral als een CT-scan van het hoofd geen hematoom laat zien),
• - Actieve bloeding, levensbedreigend of in een kritisch orgaan/gebied;
• OF bekende ernstige bloedingsneiging met eerdere bloedingen die aan deze criteria voldoen.
• Reperfusietherapie (systemische thrombolyse, chirurgische trombectomie of CDT/andere kathetergeleide therapie), of plaatsing van een vena-cava-filter voor een acute longembolie in de afgelopen 3 maanden.
• Een transporterend stolsel door een patent foramen ovale.
• Bekende Chronische Thrombo-embolische Pulmonale Hypertensie (CTEPH), of sterke verdenking op CTEPH op basis van bestaande klinische bevindingen en combinaties van tekenen van chronische LE op echocardiografie en/of CTPA.
• Bekende overgevoeligheid voor systemische thrombolyse, heparine, of een van de hulpstoffen.
• Als, naar het oordeel van de onderzoeker, of na overleg met het lokale PERT-team of EC-leden, de patiënt niet geschikt is voor trombectomie.
• Chronisch gebruik van volledige dosis orale of parenterale anticoagulantia vóór presentatie.
• Zwangerschap
• Huidige deelname aan een andere studie die de deelname aan deze studie zou kunnen beïnvloeden.
• Eerdere deelname aan deze studie.
• Weigering van uitgestelde toestemming door de naasten of door de patiënt zelf voor het gebruik van de gegevens. Uitgestelde toestemming wordt niet gevraagd aan de familie van patiënten die ter plaatse overlijden, maar wel zijn opgenomen in de studie.
