Indications

  • Injections are performed for suspected mechanical facet-mediated back pain
  • Patients with a history of more than 3 months of persistent non-radicular pain or cervicogenic headache, functional disability, not responding to conservative management and/or physical therapy
  • Injections may also be performed as an empiric treatment for persistent spinal pain not responsive to other treatment modalities e.g. nerve root, epidural, disc injection or vertebroplasty
  • Diagnostic blocks are indicated prior to definitive treatment (neurotomy), some authors recommending two, using a different anaesthetic on each occasion

  • Overlying skin sepsis
  • Systemic infection
  • Uncorrected coagulopathy (International Normalised Ratio (INR) >1.5, platelets <50)
  • Pregnancy
  • Severe allergy to proposed agents
  • Maximum steroid dose reached

Post-op

The patient should be advised that they will not be permitted to drive after the procedure as a result of potential proprioceptive and motor effects and thus, they should be accompanied by someone who can drive them home.
If a patient has a solely diagnostic block, they should not take their usual painkillers to assess the immediate response to injection better. Their pre-procedural pain should be documented using a visual analogue scale (VAS) and, ideally, some measurement of their degree of disability, e.g. the Oswestry Spine Disability Index.
If the joint is very degenerate, one may not be able to enter the joint; in these circumstances, an extra-articular, perifacetal block will often provide great benefit (the procedure may then be rescheduled under CT guidance).

 

Follow-up

Repeat indications. By consensus: one therapeutic facet joint injection per level affected, per year is reasonable if each injection provides >50% sustained relief, lasting more than three months, and RFA is contraindicated or refused by the patient. Repeating the injections at this frequency is also deemed appropriate if there has been prior posterior fusion where the medial branch nerve supply is inaccessible secondary to hardware or bone graft.
